How can I schedule an appointment?

We are starting on an emergent basis.  Fill out the contact form below and someone will respond.  Email requests will get the fastest response time.    Fax number will be added soon for referrals.    Do not email health information or documents, as it is not HIPAA compliant.

What should I expect during my first visit?

During your first visit, you will undergo an evaluation, including a discussion of your medical history and current pain issues. This helps us create a personalized treatment plan.

Do you accept insurance?

Not at start up, but we plan to do so within six months.  We will update the website with participating plans as they are approved.  The good news is that without insurance, we can offer telehealth visits when deemed appropriate by the provider.
